Job Description:

The Discharge Care Manager (DCM) is responsible for assessing barriers to a safe and efficient discharge process, identifying the relevant resources, and finally coordinating and driving an efficient discharge process with both internal and external stakeholders

Job Responsibilities:

  • The DCM RN will initiate proactive outreaches to facility CM/UM/Discharge staff upon generating or receiving a case referral to manage the discharge, working collaboratively with the facility discharging staff to assist in resolving any discharge barriers and continuing to engage facility staff until the actual discharge
  • Other tasks as requested by their manager
  • Continuously look for opportunities to improve and streamline DCM process, to maximize discharge efficacy & efficiency
  • The DCM RN will prepare and attend team huddle each morning to prepare the discharge census and prioritize patients/discharges/issues/facilities
  • Cultivate and build relationships, especially where one does not exist or is challenging, with hospitals and their personnel to build a collaborative discharge management relationship
  • The DCM RN will prepare the weekend census and potential discharges, being astute & observant to anticipate potential discharges and barrier during the weekends and aim to resolve them ahead of time
  • Collaborate closely with internal and external partners to aggressively drive safe discharges to achieve higher daily discharge volume and overall lower LOS

Job Requirements:

  • Provides guidance and serves as a resource for others
  • Solid communication skills
  • Be persistent in the pursuit of information while still providing a superior customer service experience
  • Actively contribute to the growth and the development of the team and project
  • Working in a fast-paced environment where you respond to needs of many people at the same time
  • Current and unrestricted RN license in the state of CA
  • Excellent time management skills
  • Proficient in Microsoft applications such as Word, Outlook, Excel

Qualification & Experience:

  • 3+ years clinical nursing experience in Discharge Care Management
