Job Description:
The Discharge Care Manager (DCM) is responsible for assessing barriers to a safe and efficient discharge process, identifying the relevant resources, and finally coordinating and driving an efficient discharge process with both internal and external stakeholders
Job Responsibilities:
- The DCM RN will initiate proactive outreaches to facility CM/UM/Discharge staff upon generating or receiving a case referral to manage the discharge, working collaboratively with the facility discharging staff to assist in resolving any discharge barriers and continuing to engage facility staff until the actual discharge
- Other tasks as requested by their manager
- Continuously look for opportunities to improve and streamline DCM process, to maximize discharge efficacy & efficiency
- The DCM RN will prepare and attend team huddle each morning to prepare the discharge census and prioritize patients/discharges/issues/facilities
- Cultivate and build relationships, especially where one does not exist or is challenging, with hospitals and their personnel to build a collaborative discharge management relationship
- The DCM RN will prepare the weekend census and potential discharges, being astute & observant to anticipate potential discharges and barrier during the weekends and aim to resolve them ahead of time
- Collaborate closely with internal and external partners to aggressively drive safe discharges to achieve higher daily discharge volume and overall lower LOS
Job Requirements:
- Being motivated and self-directed in driving and achieving the stated goals of the team, even when the work process is not fully defined
- Provides guidance and serves as a resource for others
- Solid communication skills
- Be persistent in the pursuit of information while still providing a superior customer service experience
- Actively contribute to the growth and the development of the team and project
- Working in a fast-paced environment where you respond to needs of many people at the same time
- Current and unrestricted RN license in the state of CA
- Excellent time management skills
- Proficient in Microsoft applications such as Word, Outlook, Excel
Qualification & Experience:
- 3+ years clinical nursing experience in Discharge Care Management
